In consideration of accepting this volunteer position, I understand and agree to be legally bound hereby for myself, my heirs, executors & administrators, successors & assigns and waive, release & hold harmless Rally For Rebecca 5K Walk, Run & Roll Plus Fun Run, elitefeats, Inc. and all sponsors and their agents, employees, representatives & assigns for any and all injuries, claims, liabilities & causes of action suffered by me in this event. I attest I am physically fit & have sufficiently trained for completion of this event and my condition has been verified by a licensed medical doctor. If signed by a parent the parent agrees to release & hold harmless on behalf of the entrant.

I grant permission to any of the foregoing organizations to take & use photographs, video and recordings or any other record of this event for any purpose whatsoever.

If signed by a parent, the parent agrees to release and hold the above-named organizations and persons harmless from any claims that may be asserted by or on behalf of the minor entrant as well as grant permission to any of the foregoing organizations to take & use photographs of said entrant, video and recordings or any other record of this event for any purpose whatsoever.
